Role of Injection therapy in Osteo Arthritic Knee


Knee pain of old age is mostly due to Osteoarthritis and its debilitating condition.

There's role of Weight reduction, Physiotherapy and lifestyle modification in decreasing the pain.

When the stage of knee arthritis is second or third when still operation isn't indicated or patient is not willing for operation there's come the role of injection therapy in knee joint.

Various types of injection therapy available are 

Intra articular steroid 

Lubrication therapy (Hyaluronic acid) 

Biological Therapy ( Platelet rich plasma -PRP)

All these injection given by specialist knee surgeon and its given in operation theater under strict aseptic precautions as any invasive procedure has chance of infection.

Steroid has been given since long and it has shown beneficial effects in early stage and for short term from 6 months to 2 years. But main disadvantage is it increases the rate of osteophyte development and long term effects are not well known  and in patient with diabetes its always better to be given when patient's blood sugar is in control.

Hyaluronic acid is Lubrication therapy it gives extra lubrication to joints so the bone surface rubbing against each other reduces to some extent and pain get decreased. It decreases the knee pain for various duration according to patient profile but doesn't guarantee much.

Biological Therapy (PRP) is the newest addition to Oa knee management and it has given promising results without any side effects in studies , PRP is prepared by specialized techniques in laboratory with patient's own blood and processed and prepared with aseptic condition. It has biological factor that will regenerate the damaged cartilage and meniscus that's Theory behind use of PRP.

So in conclusion Injection therapy is useful in mild to moderate Osteoarthritis and in patients who are still not willing and who isn't fit to undergo surgery. Its without any adverse effects so it can be tried during this covid times till everythings get settle down.

Heel Pain - How to Deal with it??

In day to day routine outpatient clinics 20% patient presents with heel pain issues ; and it's most troubling the person as walking , running and even day to day activities becomes difficult & painful. Most common cause of Heel pain is plantar fasciitis and we will see plantar fasciitis how to deal with it and get it relieved. Plantar fasciitis is as the name suggests inflammation of plantar fascia of heel and most commonly seen in females. Presenting complaints will be heel pain which is more in the morning while getting out of the bed , as day passes stiffness goes down. Pain is typical of aching pain which disturbs all activities and gradually if proper care not taken it can be more troublesome.  Exact cause of the problem is not known but walking on rough surfaces increase the problem.
